This form is to be completed for any student(s) who expects to return to school AFTER 12:00 pm Wednesday, September 9, 2026, and ON/BEFORE Friday, September 11, 2026. Please complete this form and email it macneill@sd38.bc.ca before June 24, 2026. Please attach a letter that contains an explanation of the extenuating circumstances for returning late in September.

If a student is not returning on or before September 11, 2026, they may lose their placement and may have to attend another school upon their late return.
