Graduation Transitions

All grade 12 students are required to complete the Graduation Transitions program as part of the Ministry requirements for graduation!  Upon successful completion, students receive a "Requirement Met" mark and earn 4 credits towards graduation.  Graduation Transitions is composed of the following aspects:

PERSONAL HEALTH:

Students must demonstrate the knowledge, attitudes, and positive habits needed to be healthy individuals, responsible for their own physical and emotional well-being, by completing the following:

•Developing a long-term healthy living plan that describes:
   a) Sound Nutritional Habits
   b) Regular Exercise Routines
   c) Emotional Health Management
   d) Positive Health Choices
 
COMMUNITY CONNECTIONS:

Students must demonstrate the skills required to work effectively and safelt with others and to succeed as individual and collaborative workers by completing the following:

•Particpate in 30 Hours of work/volunteer experience and/or community service and collect evidence to support experience
•Reflect on work/volunteer experience by completing the 4-page reflection sheet
•Complete the worksheet - Transferring Skills Acquired to Future Experiences
•Prepare a current resume (Instructions on how to write a resume and sample resumes)
 
CAREER AND LIFE:

Students must demonstrate the confidence and competency needed to be self-directed individuals, by completing the following:

•Completing "Career Cruising" assignment
•Completing an Transition Plan Assignment
•Completing a financial plan for the first year after high-school

PORTFOLIO PRESENTATION:

Students must present their Graduation Transition Portfolio to their Advisory Teacher.
